EBAC^® accreditation of CME activities requires all participants
to complete an evaluation form after attending an activity.

Therefore, your provider (the organization responsible for organizing and conducting the CME activity) will share a QR code or URL to access the questionnaire at the end of the activity.

After completing the questionnaire, participants will receive a unique participation code. With this participation code, you can claim your certificate of participation from the provider.

It’s important to remember your participation code, as it cannot be retrieved once the window is closed.
